Health Savings Account (HSA) Contributions

If you enroll in the PPO Saver w/HSA Plan, you may elect and contribute to a Health Savings Account. You may contribute $4,150 as an individual or $8,300 as a family to this account.

The 401(k) Plan

Traditional & Roth

Putting money aside for your retirement is easy with the 401(k) Retirement Savings Plan from ADP. The Paper Store will match up to 25% of the first 4% of your contribution.

An employee is vested at 20% once eligible for the plan, and is 100% vested after meeting the 1,000-hour requirement each year for a total of five years.

Once eligible, you will receive plan information from ADP 30 days prior being auto enrolled. The auto-enrollment contribution rate is 1%. If no action is taken, the auto-enrollment contribution will increase an additional 1% to a maximum of 2%.

Eligibility:

✓ 21 years of age on the next plan entry date


✓ Completion of 12 months of service by the next plan entry date


✓ Completion of 1,000 hours in 12 months of service by the next plan entry date


✓ Part-Time Employees: Completion of 500+ hours in 36 months of service by the next plan entry date

The Paper Store permits insurers to offer employees of The Paper Store certain voluntary insurance programs. Whether you choose to enroll in any of these programs is completely optional and voluntary. The Paper Store does not make a contribution towards the cost of these programs and employees pay the full cost of premiums on an after-tax basis. The Paper Store does not sponsor, maintain, endorse, recommend, or promote these voluntary programs. The Paper Store'S involvement regarding these voluntary insurance programs is strictly limited to allowing the insurer access to employees to publicize these programs and The Paper Store may perform certain ministerial functions such as payroll deduction and forwarding employee premium payments to the insurer. The Paper Store does not receive any consideration in the form of cash or otherwise in connection with the program, other than reasonable compensation, excluding any profit, for administrative services actually rendered in connection with payroll deductions. Accordingly, these voluntary insurance programs are not subject to ERISA and related regulations. All questions or claims regarding these programs should be directed to the insurer.

All benefit plans are governed by master policies, contracts, and plan documents. In the event of any inconsistency between the information contained herein and the applicable plan documents, the provisions of the plan documents shall prevail. The Paper Store reserves the right to amend, suspend or terminate any benefit plan, in whole or in part, at any time. The authority to make such changes rests with the Plan Administrator.
